If you are thinking about making an appointment, we offer a 20 minute complimentary consultation. This consultation is designed to give prospective patients an opportunity to meet us, see our treatment space, and learn about how Chinese medicine works.  This consultation does not include diagnosis or specific health care advice but instead is focused on education and allowing time for the prospective patient to interview Elie Cole or Danielle Lombardi.

If you have already made an appointment, we look forward to meeting you.  First, please download, print, and fill out the health history form, the consent form, the privacy form, and the financial agreement form.  Remember to bring them with you to your first appointment.  

As a courtesy to our patients, our staff will call your insurance company to inquire about your acupuncture benefits.  Please let us know if you are interested in using your insurance when you make the appointment.
